In-Home Support

Supported Independent Living

Our Supported Independent Living offers more than just a place to stay. It’s a living space that reflects your independence, while making sure the right support is close by. We help with daily routines like meals, medication, personal care, and keeping your space running smoothly, while also encouraging you to make your own choices and set your own pace. 

You’ll be supported to build confidence, develop life skills, and feel at home in a space that feels safe, respectful, and genuinely yours.

In-Home Support

Assistance with Daily Personal Activities

Getting ready for the day should feel simple and comfortable. We provide one-on-one assistance with personal care tasks like showering, grooming, dressing, and toileting, with support that is always respectful and adapted to how you like things done. 

Our goal is to make sure your routine is supported in a way that protects your dignity, maintains your independence, and helps you feel confident in your own space.

In-Home Support

Community Nursing Care

Our community nursing service brings experienced, professional care into the home. We support people with complex or ongoing health needs through services such as medication assistance, wound care, health monitoring, and help managing chronic conditions. 

Everything is done with a focus on comfort, privacy, and working closely with your existing care team. We take the time to get to know you, understand your health needs, and offer care that is consistent, reliable, and reassuring.

In-Home Support

Household Tasks

Keeping on top of the household can be hard without the right support, and that is where we come in. We can assist with regular tasks like cleaning, laundry, meal preparation, and shopping.

Whether you need someone to take over a task completely or just lend a hand, we tailor our help to fit your preferences and needs. The aim is to keep your home clean, comfortable, and running in a way that suits you, while freeing up time and energy for other parts of life.

Capacity building

Support Coordination

We strive to empower participants to make informed decisions about their NDIS plans, promoting choice and control over their supports.

Our Support Coordination service is designed to guide and assist individuals in navigating the NDIS system.

Our dedicated support coordinators work closely with participants and their families to identify goals, access appropriate services, and build a comprehensive support network

Capacity building

Building life skills is not just about learning tasks, it is about growing confidence and independence. We work with you to build skills in areas like cooking, communication, using public transport, managing money, or anything else that supports your goals.

Capacity building

Specialised Positive Behaviour Supports

We understand that behaviour is always connected to communication, environment, and stress. Our behaviour support team works with you and the people around you to develop tailored strategies that help reduce distress and build positive outcomes. 

Through respectful observation, clear planning, and consistent support, we focus on understanding what works for you and creating safer, more supportive environments. Plans are developed in partnership with you and reviewed regularly so they stay useful and effective over time.

Capacity building

Access and Maintain Employment or Higher Education

Finding or keeping a job or pursuing education can be challenging without the right support. We help you prepare for work or study by building skills like time management, CV writing, interview preparation, and confidence in navigating systems and settings. 

We can also support you while you’re working or studying, helping to manage routines, problem-solve, and stay on track with your goals. Our focus is on creating real opportunities that reflect your interests, strengths, and future plans.

Community Access

Community Participation

Being part of the community is about more than just getting out of the house. We support you to connect with activities, events, and groups that match your interests and help you feel included and confident. 

Whether you want to join a class, catch up with friends, attend a local event, or simply go to a café, we can be there to help you plan it, get there, and feel supported while you’re out.

Innovative Community Participation

We offer creative and meaningful ways to explore new interests, meet people, and try different activities. This can include art programs, volunteering, skill workshops, or other experiences that help build confidence and personal growth. 

Innovative community participation is about stepping outside of the usual, in a way that is safe, supported, and always led by what you feel ready for and excited to try.

Community Access

Travel and Transport Arrangements

Getting from place to place is easier with the right support. We help organise and provide transport so you can get to medical appointments, social activities, education, or anywhere else you need to be. 

Whether it’s regular trips or occasional outings, we make sure transport is reliable, safe, and arranged around your schedule.
